Session 4.3 - Presentation & Discussion: Best EP Practices for the Future: PFA Should Not Be Performed on All Heart Failure Patients with AF
This session, chaired by Douglas Packer (Murray, US), reviews best EP practices for atrial fibrillation in heart failure, framing management around four pillars of AF therapy and four pillars of heart failure therapy, and comparing drug treatment with ablation, including data from CASTLE-AF, CABANA and meta-analyses that support catheter ablation, particularly in HFrEF, while noting that PFA is promising but not yet clearly superior or appropriate for all patients due to evolving data on durability and complications. In the discussion, Nassir Marrouche (New Orleans, US) and Amin Al-Ahmad (Austin, US) highlight the need for cautious, targeted PFA lesion sets and thoughtful catheter selection in structurally advanced disease, while Packer and moderator Devi Nair (Jonesboro, US) emphasises that, despite powerful new mapping systems and energy sources, EPs must not abandon core electrophysiology principles or critical thinking when treating heart failure patients with AF.
