Easy and Safe PFA PVI With a Circular Over-the-Wire Array Catheter

The session, chaired by Devi Nair (Arkansas, US), featured a live pulmonary vein isolation (PVI) procedure using a circular over-the-wire array catheter for pulsed-field ablation (PFA). Case operator Lucas Boersma (Nieuwegein, NL) guided the audience through the procedure, detailing key steps such as transseptal puncture with the FlexCath Cross, catheter manipulation, and ablation strategy. The panel, including Andreas Metzner (Hamburg, DE) and Suneet Mittal (New York, US), discussed procedural techniques, catheter positioning, and patient selection. Dr Boersma demonstrated how the catheter’s over-the-wire design ensures stability and optimal contact with the pulmonary vein ostium, facilitating precise lesion delivery.

Key topics included safety considerations, fluoroscopy versus mapping approaches, and procedural efficiency, with most cases completed in under 45 minutes. The discussion also addressed pulmonary vein stenosis risk, discharge protocols and lesion durability. The session concluded with a review of the ablation’s effectiveness, confirming complete isolation of all targeted veins. Faculty praised the streamlined workflow and efficiency of PFA.
