Session 1.3 – Live Case & Discussion: PVI with Balloon-in-Basket Technology: Efficient and Straightforward

This session showcases a live case from Abdul Parwani and Hendrik Boldt (German Heart Centre Berlin, Berlin, Germany), focusing on pulmonary vein isolation using the VOLT PFA system with balloon-and-basket catheter technology. The team treat a 71-year-old man with symptomatic paroxysmal AF, intolerant to drug therapy, highlighting a streamlined workflow without pre-procedural CT and using ultrasound-guided access and NAPS sedation.

The operators demonstrate plug-and-play catheter preparation, impedance-based contact assessment, “mapping on the fly” and stepwise energy delivery. Discussion centres on optimising contact, number of applications per vein, and fluoroscopy-only workflows. Moderator Richard Schilling (London, UK) and the panel – Roland Tilz (Lübeck, Germany), Boris Schmidt (Frankfurt, Germany), Daniel Rodríguez (Madrid, Spain) and Devi Nair (Jonesboro, US) – debate imaging strategies, angiography, SVC and posterior wall ablation and durability considerations.

The case concludes with rapid isolation of all four veins and confirmation of block, reinforcing the efficiency and practicality of balloon PFA for straightforward PVI.
