Session 2.8 – Future Directions in Leadless Cardiac Rhythm Management
This session explores how leadless pacing is evolving from single-chamber support to truly modular, long-term cardiac rhythm solutions. Led by chair Richard Schilling (London, UK) with a keynote from Vivek Reddy (New York, US) and expert input from Devi Nair (Jonesboro, US) and Mikhael El-Chami (Atlanta, US), the discussion reviews dual-chamber leadless pacing, battery longevity challenges and the emerging role of energy-harvesting pacemaker technology. Early data on piezoelectric and electrochemical systems are highlighted, alongside concepts such as bioresorbable and temporary leadless pacing options.
The faculty also examine how to minimise pacing-induced cardiomyopathy and optimise patient selection for leadless systems. Practical strategies include combining leadless right ventricular devices with left ventricular stimulation technologies, targeting left bundle area pacing and adopting atrial-only leadless pacing for patients with sinus node dysfunction. The panel share real-world experience on cost considerations, device extraction, modular upgrade pathways and how these innovations may reshape standard practice in both US and UK centres.
