Session 1.4 – Live Case & Discussion: Streamlined PFA Workflow for AF Ablation

This live case from Christian Sohns (Heart and Diabetes Center NRW, Bad Oeynhausen, Germany) highlighted how a streamlined pulse field ablation workflow can deliver rapid, efficient PVI without compromising control or safety. Treating a 57-year-old patient with paroxysmal AF, the operators demonstrated single-catheter mapping and ablation using the VARIPULSE system, showing how real-time Tissue Proximity Index (TPI) supports confident catheter–tissue contact and targeted lesion delivery. The pulmonary veins were successfully isolated within minutes, with the panel discussing considerations including irrigation strategy, avoiding overlapping electrodes, haemolysis risk, contact assessment and the implications for redo procedures.

Throughout the case, the faculty compared PFA technologies and shared perspectives on durability, posterior wall ablation, workflows without general anaesthesia and the growing role of same-day discharge. The session offered practical insights for centres seeking to integrate PFA into routine AF ablation with speed, reproducibility and precision.
