This plenary session features a live case from Julian Chun (CCB Markus Krankenhaus, Frankfurt, DE), demonstrating a streamlined workflow combining pulse field ablation (PFA) with left atrial appendage (LAA) occlusion in a single procedure. The case involved a symptomatic 75-year-old patient with persistent atrial fibrillation, heart failure and haemophilia B, providing a real-world scenario where rhythm control and bleeding risk must be balanced. Chaired by Rod Tung (Phoenix, US) – and with a faculty of Bradley Knight (Chicago, US), Poojita Shivamurthy (Philadelphia, US) and Rodney Horton (Austin, US) – the case entails a step-by-step walk through pulmonary vein isolation using a pentaspline PFA system under nurse-assisted propofol sedation (NAPS), followed by device sizing and implantation for LAA closure. Expert panellists offer practical insights on case selection, transseptal technique, device choice, posterior wall considerations, and anticoagulation strategy post-procedure.
